Tutoring Referral from a Caseworker 

 

Tidewater Friends of Foster Care provides Individualized Tutoring Free of Charge for students referred for services by our Partner Service Agencies (with an existing MOU in place). 

If you would like to refer a Student on your case load for tutoring services, please complete the Referral form below.  Once you have submitted this referral, we will send the Student’s Caregiver/Guardian a Tutoring Request Form.  The Tutoring Request Form MUST be COMPLETED by the Caregiver/Guardian for Services to begin.  Thank you!

Caseworker Referral: TFFC provides one-on-one, professional tutoring for students referred through Partner Service Agencies.

Approval of services is required and is based on eligibility and available funding. A tutoring application form will be emailed to the caregiver/guardian you list below.

  • Referring Entity

  • A Partner Service Agency must have an existing MOU with TFFC to be eligible for referring students for tutoring.

  • Tutoring Guidelines

    • A Responsible Adult must be present for all tutoring sessions and is responsible for providing a suitable work environment, as well as materials and supplies. • Cancellation or rescheduling of tutoring sessions requires 24-hour notice to the tutor, otherwise the session is considered a No Show. • After three (3) No Shows, tutoring will be suspended and the caregiver will be required to pay a $25 fee to reinitiate services. • Students remain eligible for tutoring through the end of the academic year. • Following each Tutoring Session, a Session Report is provided to the referring worker and the caregiver as evidence of services rendered.

  • Hidden
    Tidewater Friends of Foster Care, Inc. has put in place Virtual Tutoring to reduce the spread of COVID-19. Participating in In-Person Tutoring provided by TFFC could increase your risk and your child(ren)’s risk of contracting COVID-19.

  • Caregiver/Guardian Information

    Please enter the name of the parent, caregiver or guardian who will receive the tutoring request form. Note, this person must be legally authorized to sign a Consent to Exchange Information between TFFC and the Student's School.
  • The Tutoring Information Request Form will be emailed to the Caregiver/Guardian upon Submission of this Referral.

  • Student Information

    We will collect the remaining information from the Caregiver/Guardian once you have submitted the form.
  • Why do we ask for Race? Many of the foundations who fund the TFFC Tutoring Program wish to understand the impact they are having among people and communities of color. We use demographic data to show the racial diversity of our program participation.
